How to Create HTML Form

Boss need me to create a customer feedback form for her, she need that for her online store business. I'm worry now, I don't even know how to use html code then how could I create the HTML form for her? She though I know everything about creating website? Oh my. I only know blogging which doesn't need any HTML skill!

Lucky I got many online friends that can recommend something good to me. Haha...One of my online friends introduce the forms4free website to me. He said this website is provided free form which can create by us without hassle. All form can be done in 5 minutes and they will email the form to us after that. OK...I will try this out and see is that true or not. haha
